Gift Aid Declaration

By ticking the box, I agree to The Mainstay Foundation (charity registration number in England and Wales: 1166017) claiming Gift Aid on the donations I have made within the last four years prior to this declaration and all donations I make from the date of this declaration until I notify The Mainstay Foundation otherwise.

I understand that The Mainstay Foundation will reclaim 25p for every £1 that I donate or have donated in the last four years.

I am a UK taxpayer and understand that if I pay less Income Tax and/or Capital Gains Tax than the amount of Gift Aid claimed on all my donations in that tax year it is my responsibility to pay any difference.
